Quick Start Extra Credit does not add form entry to student records

Hello, even after re-doing the block which calculates age based on grade, entering a record into the form and hitting submit does not lead to the entered record being displayed.

Also the submit button is on the same line as the last form element as this screenshot shows:

Sorry about that, posted the solution here: https://github.com/witheve/Eve/issues/486 and fixed it for anyone new coming to the quickstart. It won't automatically be fixed if you've made any changes to the quickstart because we didn't want to overwrite things you've done.

We'll add something that allows you to reset back to the original example monday.
