ETP Prototype/Model Submission

on 26th October or 28th October 2010

All ETP groups have to submit their respective ETP model/prototype to Mechanical Engineering

Department before July 2010 semester ends.

 

The venue that each group can submit their ETP model/prototype is ETP Store Room located at

17-02-19. The technicians in charge for the submission of ETP model/Prototype are:

 

1.       Mr. Faisal &

 

2.       Mr. Mahfuz

They can be contacted at the number 05 368 7019.

The 2 days allocated for ETP model/prototype submission are Tuesday, 26^th Oct and Thursday, 28^th Oct

2010. The time for submission allocated on the 2 days is from 8.00 am to 5.00 pm.

Please contact one of the technicians above (Mr. Faisal or Mr. Mahfuz) for arrangement before

 your ETP model/prototype submission.

ETP CLAIM

 

Dear all ETP Groups,

 

Your groups’ requisitions for ETP as written in FORM 03 (APPENDIX XVII: STUDENTS’

 REQUISITION FORM) have been approved by Mechanical Engineering Head of Department.

 

The next thing you need to do is to fill up 2 forms:

1.       ETP CLAIM BREAKDOWN FORM

 

2.       UTP CLAIM FORM

 

The UTP CLAIM FORM will be provided by me. The sequence of writing the items in the above-

mentioned 2 forms will be the same like the sequence written in FORM 03. Please come and

meet me to:

 

1.    get back the approved FORM 03 so that it can serve as a reference for you to fill out ETP

 

     CLAIM BREAKDOWN FORM and UTP CLAIM FORM

 

2.     obtain the UTP CLAIM FORM

 

Please send any representative from your ETP group to come and meet me this Friday, 22

October 2010, anytime from 9 am to 5 pm.

 

Please have a look at Figure 1: ETP Purchase Requisition Flow Chart in page 11 of the ETP

Guidelines about the main steps involved. Please make sure that Step 5 – Arrange original

receipts according to sequence in FORM 03, paste original receipts on A4 papers is adhered to.

 

For further explanation on the procedure, students can refer to Table 2: Explanation to ETP

Purchase Requisition Flow Chart in page 12 of the ETP Guidelines. Referring to Item 6 in Table

2: Explanation to ETP Purchase Requisition Flow Chart, remember to request your respective

supervisors to fill out his/her details in the UTP CLAIM FORM. Please take note and follow the

deadlines strictly.

 

You will need to submit 4 things to me by 4^th November 2010:

 

 

1.       ETP CLAIM BREAKDOWN FORM

 

2.       UTP CLAIM FORM

 

3.       Original receipts pasted on A4 papers

 

4.       Original FORM 03 (and Justification Letter is  applicable)  that has been approved by

      Mechanical Engineering Head of Department
