Importing table to Database file

I'm hoping someone can help. I'm trying to import our list of databases into the ERMes Database file and keep getting an error message that says the field 'Source' doesn't exist in the destination file. I first exported the Database file to populate the fields and am just trying to export the same file back but with the information from our system filled in. 

 

Anyone have any tips?
