Not able to upload attachment and reply to the Comments from Eclipse

Dear,

I am not able to upload attachment from Eclipse. It is giving me the option to upload the file. But, It is not getting updated. The uploaded image or file is not getting shown in the Attachments section.

Similarly, I am not able to reply to the comments given by reviewer from Eclipse.

Please help me out the fix the problem.
