Ethereal Realms 4.1.1

This rather rapid successor to the 4.1.0 version of the source is due to
a lingering issue that should have no longer existed. This new
release should fix those issues and have things work as expected.

This is a code release only, meaning people who have 4.1.0 installed
simply need to install the files over the existing code base. Headers
should be adjusted as need be of course.
