Intake Forms Guide Lines

Hello Team,
We have to clean up the way we are treating clients confidential information. During my visits, I’m finding to many intake forms being kept by the Spa computer located in the Spa Lounge.
These documents which may seem like just a piece a paper, are state mandated confidential documents that protects not only your employer from getting fines, it protects your employment and your State professional License when we are inspected.
- Intake Forms must have written Soap Notes after each session, with date and length of services. Why? It’s used as a 2nd proof of documentation that the client you are seeing, did see you, updated contact info, also in court if requested for legal matters.
- Intake forms must be filled in alphabetical order utilizing the filling system that is set up at your Spa. 92nd Street (File draw in your Spa Manager/ AGM’s office).
- Failure to follow these guidelines will be documented and lead to termination. I will not tolerate our clients who depends on us pertinent information left out for the world to see.
