This is a really annoying request but can you all please email me back this attached form with your detailed information by the end of this week? (Friday). I would very much appreciate it as it has to go to nationals. Josh was supposed to give this to you at some point when you turned in your dues, but we're taking care of it now in case you didn't fill it out yet
