The issues of Patients Identification Number for Patients who are on
Care and ART have been a big challenge, however AIDSRelief supported
Local Partners Treatment Facilities within Tanzania have been utilize
the NACP CTC2 Database System. The NACP CTC2 Database System is
designed to be a simple, useful tool for HIV/AIDS clinics in Tanzania
to store and analyze data specified for collection by the National
AIDS Control. It is designed according to the NACP standard CTC2 card/
forms and report formats. The CTC databases were designed by a
partnership between the Ministry of Health National AIDS Control
Programme and the University Computing Center under support from the
Global Fund Tanzania. HIV/AIDS Partners have engineer on the system
updating and requirements. In Tanzania we are using 14 digits
Patients Identification Numbering System in order to assist us in
assigning Patients Unique ID Number for CTC Patients. This has helped
us in tracking HIV/AIDS Patients movements among various regions
within Tanzania. The HIV/AIDS Patients can be Transfer Out to another
facility and accepted to other facility with records.

In additional through the use of IQTools package(Software for Data
Cleaning, Validation and Reporting-Developed by Futures Group ) the
health facilities can able to get update list of all Transfer In
Patients detected/recorder from other health facilities which help/
assist them in update patients who were treated as “Lost To Follow
Up” or “Unknown” within the system while have been found are
receiving HIV/AIDS services in other health sites

Through the use of this utility tool we have manage to track HIV/AIDS
Patients Movements among AIDSRelief supported Local Partner Treated
Facilities, Specific we have manage to provide number of Transfer Out
Patients who were on Care and for those who are on ART. In additional
we have managed to track the Transfer In Patients who are coming from
other health facilities. As a results this IQTools assist hospital
staff to identify true defaults within their program



As results HIV/AIDS patients have a guarantee and can continue
receiving Care and ART to other health facilities provided he/she has
referral form (include CTC ID Number(14 digits) ) to other health
facilities .



The 1st two digits represents region, 2nd two digits represents
district, 3rd two digits represents facility name, 4th two digits
health center, 5th six digits Patients ID Number

Example 21-02-0100-000067: Represents (21)1st, Region ,(02)2nd,
District (01)3rd =Facility Name, (00)4th =Health Center, (000067)5th
=Patient ID Number.



Through the numbering system each Health facility can able to track
patients movements (TO/TI) within their health center and also among
various health facilities.

Dear Essau,

This will be a very interesting system to see. I realise in Kenya we
also use a similar ID system. We have lots of problems with
defaulters, many of whom are not actually true defaulters but seek
care elsewhere, mostly due to stigma.
