I hope after we attend WHO workshop held at Tanzania and through sharing various EMR system implemented at various Countries for Patient Management Monitoring System and for tracking the Termination ART Patients. However most of Health facilities have realizes the need for promoting Data Use Culture in their health facilities and in additional the Data Demand Information Use became to do list and necessity Monitoring and Evaluation activities in most of health facilities which are providing HIV/AIDS, TB/HIV and PMTCT services.
As a way forward Strategic Information Team within Futures Group at Tanzania have establish and strengthening Patient Management Monitoring System for the HIV/AIDS Patients who are on Care and for those on ART through identify Indicators in order to track HIV/AIDS Patients Progress in Term of CD4 gain, ARVs initiation and Clinical Stage criteria for ARVs initiation among 51 Computerized Health facilities.
Through the use of IQTools software package for data validation (ARV patients: Over 90 days since ARV ran out and ARV patients: >90 days since Next App) and for Clinical reporting purpose by linking within IQTools with CTC2 Database (used by NACP –Tanzania as national system) we have manage to identify the list of Patients who are on “Waiting List” .i.e. Eligibility CD4 <200 but not started ARVs and for those Eligibility: WHO stage 4 but not started ARV (over 14 days). Through the IQTools we have able to provide a list of HIV/AIDS patients and share it with Clinician Team within Health facility for close follow-up.
Through the use of CTC2 database SI Team have promote the health facility to conduct CD4 Analysis through providing a list of HIV/AIDS patients with missing CD4 count who are suppose to undergo baseline and 6/12 months follow-up CD4 count based on their next appointment. This list have been shared among staff working at CTC for action .As a results the number of patients with baseline and follow-up CD4 have increase dramatically and its now possible to do CD4 Cohort Analysis in order to determine the quality of Care provided within CTC Unit
So far SI Team is piloting IQCare System within two Health facilities at Tanga region, IQCare (International Quality Care) is a patient management system designed to meet the data collection and management needs of the AIDSRelief program in Tanzania. The objective of IQCare is to help facilities build their capacity for enhanced data analysis and use. IQCare provides the ability for local partner treatment facilities (LPTFs) to customize the software based on their needs and their clinical environment
