New issue 305 by antonio.petrelli: XmppRoster fails to identify refusal or
removal of subscriptions when contact not in roster

If the contact is not in roster and a refusal of subscription, or a removal
of a roster item, arrives, the roster item is *added*.
It should be removed instead.
