getting error while validating the CCDA with 170.315_b1_ToC_Inpatient_Sample_1

Hi team, 
getting error while validatoing with  170.315_b1_TOC_Inp sender.

we got 3 errors and found the data in the CDA alsoas per the IG for uscdi v3.

All have data as per the IG, kindly updated us what is wrong.

please find the attached files:
1. validator options we selected on ETT Tool.
2. sample CDA having data.(170.315_b1_ToC_Inpatient_Sample_1_(v1)_new6.xml)
3. Validator error pdf having 3 validator errors.

3 errors.

1. 238131007 - Overweight (finding) - The error says no Health Concern Act, BUT test data has in “USCDI Data Class/Element: Problems”. I believe this will require either a fix in the test data or tool. When I move to Health Concern it passes. Dragon can update on planned update.

2. Disability Status Observation - Check your LOINC code.
