I am currently validating the C-CDA R2.1 V4 TOC Inpatient – Scenario 1 using the C-CDA Validator. While validating the document, I am encountering the same validation error across multiple scenarios:
Result Description: Facility information for 1160-1 , for EncounterActivity data for code D63.1 : The scenario requires FacilityIdentifiers data, but submitted file does have FacilityIdentifiers data.
The message is confusing because it indicates that the scenario requires FacilityIdentifiers data, while also stating that the submitted file does have FacilityIdentifiers data.
Has anyone encountered this issue before? Is this a validator message issue, or is there a specific requirement for the FacilityIdentifiers element that I might be missing?
Any guidance or suggestions would be greatly appreciated.

Are these validation errors caused by issues in our XML, or are they related to the C-CDA Validator on site.healthit.gov?

Could you please explain in more detail which part of the XML might be causing this validation issue?
The validator message says:
"The scenario requires FacilityIdentifiers data, but the submitted file does have FacilityIdentifiers data."
Based on this message, I can see that the FacilityIdentifiers element is already present in my XML. If there is something incorrect or missing in the XML structure, could you please point out the specific section or requirement that is not being met?
For example, is the issue related to:
The id element or its root/extension values?
The facility organization template or hierarchy?
The encounter participant/location mapping?
A mismatch between the scenario data and the XML content?
Any details or example would be very helpful. Thank you for your guidance.
