UDI organizer template ID is currently 2.16.840.1.113883.10.20.22.4.312 where it should be 2.16.840.1.113883.10.20.22.4.311. This is based on the Appendix B: UDI Organizer template from C-CDA Supplemental Templates for Unique Device Identifier (UDI) R4.

If we update the templateId root to 2.16.840.1.113883.10.20.22.4.311 it fails conformance, where 2.16.840.1.113883.10.20.22.4.312 does not fail. Can someone confirm which templateID root should be used?

Looking at your file, the issue that you are missing the required <id> element and also your statusCode is "active" when the IG says it should be "completed". If you add those and then use the .311 template ID, it does not have any errors on this section of the CCDA.
