Clarification Needed on Social History Observation Warnings

Hello Team,

I am seeing repeated validation warnings for the Social History section in the b1 – Alice Newman 1 test case.

The validator reports the following warning multiple times:

“Consol Social History Observation V4 SHALL contain exactly one [1..1] code (4515-8558), which SHOULD be selected from ValueSet Social History Type (2.16.840.1.113883.3.88.12.80.60).”

These warnings are occurring for the following Social History entries:

  • Tribal Affiliation

  • Occupation Observation

  • SDOH Assessment

Additionally, even when I remove the Social History Observation template
(templateId: 2.16.840.1.113883.10.20.22.4.38) from the Tribal Affiliation and Occupation Observation entries, the same warning continues to appear.

I am sharing:

  • A screenshot of the validator warnings

  • The Social History XML section

Could you please clarify the correct way to represent Tribal Affiliation, Occupation Observation, and SDOH Assessment in the Social History section for b1 certification, and advise how these warnings should be resolved?

This is expected behavior from the rule
Shall on the existence of the element. - Should on the value is in the valueset

in other words

Each Social History entry must be labeled as Social History.
Using the standard Social History code is strongly recommended to ensure consistency, but an alternative may be used if necessary.

What can be confusing about the standard is a perfectly reasonable CDA document instance will have Should and May - it is meant to illuminate a possible issue or best practices
