Clarification Required on C-CDA USCDI v4 Validator Test Scenarios

Hi Team,

I am currently testing our application's C-CDA XML using the Health IT C-CDA USCDI v4 Validator for ONC certification.

I have a few questions regarding the validation scenarios and would appreciate your guidance.

Questions

1. Our application supports all 20 C-CDA sections Checkbox. While validating the generated XML, I'm unsure which validation scenario should be selected for each certification criterion.
   Could someone please guide me on which C-CDA USCDI v4 validator scenario I should select for each of the following 20 C-CDA sections: Patient Demographics, Encounter, Problems, Allergies and Intolerances, Medications, Immunizations, Laboratory, Clinical Tests, Diagnostic Imaging, Procedures, Vital Signs, Clinical Notes, Health Status Assessments, Goals and Preferences, Patient Summary and Plan, Care Team Members, Health Insurance, Medical Devices, Facility Information, and Provenance? Also, could you explain which validator scenario should be used for each clinical document type (Consultation Note, History & Physical, Progress Note, Discharge Summary, and Referral Note) and provide the complete C-CDA USCDI v4 validation flow?

   For example:

   • When should I select 170.315_b1_ToC_Amb?
   • When should I select 170.315_b1_ToC_Inp?
   • When should I select 170.315_b2_CIRI_Amb?
   • When should I select 170.315_b2_CIRI_Inp?
   • Similarly, which validator option should be used for the other certification scenarios?

2. Our application can generate the following Clinical Document types:

   • Consultation Note
   • History & Physical
   • Progress Note
   • Discharge Summary
   • Referral Note

   Could you please let me know which validator scenario should be selected for each of these document types? I'm not sure whether they should be validated under ToC, CIRI, or another certification scenario.

My main question is: For each C-CDA USCDI v4 validation scenario, should I select only the relevant C-CDA section checkboxes, or should I select all 20 C-CDA section checkboxes before generating and validating the C-CDA document? If only the relevant sections should be selected, could you please provide a mapping of each validation scenario (such as ToC, CIRI, VDT, Care Plan, DS4P, etc.) to the specific C-CDA section checkboxes that are required for that scenario?

Hello Rajkumar, 

 You can select any criteria depending on your EHR setting and use case..for example, if you are ambulatory EHR and you want to test transitions of care you can select B1 ToC Amb scenarios. Ultimately we cannot give this guidance you have to work with your internal business, compliance and regulatory authorities to decide on what you want to do.
