Missing Document Level Provenance Data

I keep getting the following type Validation error for all the scenarios files when running my TOC file through the "Validate your C-CDA for USCDI v1" test.

Error - The scenario requires Document Level Provenance data of time and/or representedOrganization/name which was not found in the submitted data. The scenario time value is 20150722 and a submitted time value should at a minimum match the 8-digit date portion of the data. The scenario representedOrganization/name value is Neighborhood Physicians Practice and a submitted name should match. One or all of the prior issues exist and must be resolved.

I've looked everywhere in my TOC and I do have the document generated time set to 20150722 and representedOrganization set to Neighborhood Physicians Practice so I'm not sure why it's erroring. Where in my TOC is this test case checking to see if the document level provenance data is set correctly?

FYI the validator for Gold_Samples_for_Practice does not work. I ran an XML file for another patient through it (which should definitely result in lots of errors and it returned 0 errors). This makes it impossible to validate if the Gold_Samples_for_Practice example files even work or not.

Nvm, figured it out.

For others that may be facing the same issue, I was missing:

from the <author></author> section. It's confusing because none of the example files (that "pass" the validator) have this section in it.
