Sample Human Readable CCD Request

I'm reaching out to see if anyone has a non- PHI sample CCD in human readable format.  We're making updates for the b1_ToC_Amb and my team is trying to determine how other teams are placing SDOH items in sections.  Are we expected to create a separate SDOH section? Or should the assessments, goals, and interventions fit in existing assessment, goals, and procedures sections?
