Question patient-requested restrictions

question about patient-requested restrictions.  What is the minimum set of demographic data that is needed to correctly identify the patient and pass the ccd to the patient's portal? Beyond the medical record number, what else would be considered a requirement? Meaning, are there any demographic data elements,...patient name, dob, address, that would be impossible to allow a restriction?
