Clarification on USCDI v3 Data Element Value Sets – Sex, Gender Identity, and Sexual Orientation

Hi Team, 

We need a clarification regarding the value sets and codes for the USCDI v3 data elements – Sex, Gender Identity, and Sexual Orientation. 

While cross-referencing the C-CDA R4.1 Companion Guide (June 2023) tables with the Value Set Authority Center (VSAC) listings, we have noticed certain code mismatches and differences between the C-CDA IG code tables and the latest active VSAC value sets.

 

Details of Observed Mismatches: 

Gender Identity:  The VSAC list includes SNOMED CT code 33791000087105 (Identifies as nonbinary gender (finding)), while the C-CDA R4.1 IG lists 446131000124102 (Identifies as non-conforming gender (finding)). 

Sex:  The VSAC list includes 33791000087105 (Identifies as nonbinary gender (finding)), whereas the C-CDA IG lists 184115007 (Patient sex unknown (finding)) instead. 

Sexual Orientation:  The VSAC expansion includes codes like Asked But Declined, while the C-CDA R4.1 IG refers (Asked But Unknown)

 

Could you please confirm; For USCDI v3 implementation in C-CDA do we need to follow:

  • The latest active VSAC expansions (as per VSAC website),
  • or
  • The C-CDA R4.1 Implementation Guide tables, even if the codes differ from VSAC?

Hi Joe
  • In my opinion, you should wait for the Certification Companion Guide to be back available after the Government Shutdown
  • If you don't want to wait, try to use web.archive as a start for your target Criterion
  • Then focus on the Patient PDF Stories from the SITE Validator
  • Then when looking into VSAC, make sure you check the valueset version and compare to the C-CDA R4.1 PDF to see if they are different versions
