Is there a list of SNOMED-codified allergy reactions out there anywhere?

I see one from the CDC, but it doesn't seem all inclusive.

Any response here?

We are receiving an error for the SNOMED code we are choosing for hives. Is there a specific SNOMED code reactoin value set we are supposed to stick with?

Thanks!

Hi, Diana,

Check VSAC for the list of allowed codes:

https://vsac.nlm.nih.gov/valueset/2.16.840.1.113762.1.4.1010.9/expansion

Best,

Brett

Thanks Brett!

I downloaded the entire value set however the SNOMED code for hives is not in there...

It also looks like a list of allergies that are not drug related (not necessarily reactions).

I searched for both hives and SNOMED of 247472004. Both were not found.

is there a different value set by chance?

Sorry Diana, I sent you the substances!

See this one for reactions:  https://vsac.nlm.nih.gov/valueset/2.16.840.1.113883.3.88.12.3221.7.4/expansion

I don’t see hives present either, let me do some digging.

Best,

Brett

Good morning Diana,

I hope you had a nice weekend.

After a several emails over the weekend it appears something isn’t quite right in VSAC. 247472004 is valid.

The expansion version of 20170504 only includes 2,668 concepts, when it should be over 110k!

Update the expansion version to (2017-06-09) for the allowed terms:

The Validator needs to be updated with this full set.

Best,

Brett

Thanks Brett!

Also, does this mean there still isn't a smaller subset of the SNOMED problem value set that just represents allergy reactions?

Do EHRs hand select out of the 110K for those?

Thanks again for chasing this one down!

Hi, Diana,

I certainly hope not!

There are terminology vendors who support subsets, and some EHRs build ‘most frequent’ to avoid a full 110k search every time.

Best,

Brett

Hey Brett,

I was wondering if you had a response to a smaller subset of allergy reactions (instead of picking through a list of 110K terms...)

Also, do you know how we should represent an allergy reaction that doesnt have a SNOMED option? Such as "questionable" or "unspecified"?

See screen shot!

Thanks!

Hi, Diana,

I am cc’ing a few folks from HL7 patient care that presented on ‘most common allergens’

I haven’t received a request to code "questionable" or "unspecified” previously.

Dr. Leftwhich – Are those appropriate codes for a reaction?

Hi, Rob,

Before giving guidance on ‘questionable’ and ‘unspecified’, I was hoping Russ could confirm those are clinically valid…and provide input on how patient care approached in FHIR.

Best,

Brett

Folks,

I can’t see the screenshot so don’t know what that communicates. The “Wheal” issues is moot since that concept is in the value set.

Perhaps I’m missing something but I think this latest email was not about an actual physical reaction, but how to incorporate the following very different things:

  1. some sort of qualifier like “questionable”,
  2. Represent that the reaction is “unspecified”

Russ should answer the first issue.

The second, in this specific case, should be communicated using an infamous V3 NULL flavor. I’d assume UNK. Brett, that one should be on your list of “how to do” I think.

I will defer to Rob on the value set question, although that particular SCT code of "wheal" is indeed not specific for allergy, might or might not be a manifestation of an allergy.
