Regarding patient-requested restrictions, what is the minimum set of demographic data required to correctly identify a patient and still be able to pass the CCD to the patient portal? Beyond the medical record number, are there any additional demographic elements (such as patient name, date of birth, or address) that are mandatory? In other words, if a restriction is applied, what is the minimum patient demographic information that must still be included in the C-CDA?
