additional USCDIv3 data elements not being checked

Besides Disability Status, there are some other USCDI data elements that are not being checked in the CCDA validation for Alice Newman.

Neither Functional Status Component <templateId root = "2.16.840.1.113883.10.20.22.2.14"/> nor Mental Status Component <templateId root = "2.16.840.1.113883.10.20.22.2.56" extension = "2015-08-01"/> are being checked. Both component sections can be removed from an AliceNewman test file and it will pass scenario validation.

Also, the tool is not checking for the absence of Medication Indication (<templateId root = "2.16.840.1.113883.10.20.22.4.19"/>) or Reason for Referral Indication (<templateId root = "2.16.840.1.113883.10.20.22.4.19"/>).

Additionally, the entire Patient Referral Act (act 2.16.840.1.113883.10.20.22.4.140) is not being checked and can be omitted from the file and still pass validation.

Finally, the Clinical Test EKG (44975-1) is not being checked or flagged missing.

Ideally, it would be good if those elements were flagged if missing.
