System limitations for diagnoses errors during testing

Experiencing encounter diagnosis issues for Jeremy Bates and David Hollow scenario testing.  Our system doesn't allow for problems to be blank and a diagnosis to just be used for E/M charting. How should we address these or can we ignore these errors?
