Hey Folks,

For the second sample of the b1,b6,e1 and g9 Sender test data, the medication allergies list no known food allergies, no known environmental allergies, and no known history of drug allergy. The SNOMED codes listed with these data elements are not within the C-CDA vocabulary list for Allergy and Intolerance Type in C-CDA R2.1 or R1.1. In addition, the final rule doesn't include no known allergies as a requirement. Is this data required for certification?

Clyde,

The data is required for certification.

The Rule talks about representing C-CDA according to the HL7 C-CDA IG R2.1 and also to follow the best practices from the HL7 Example Task Force.

http://wiki.hl7.org/index.php?title=CDA_Example_Task_Force

Also the Allergy and Intolerance Type Value Set (2.16.840.1.113883.3.88.12.3221.6.2) does allow SNOMED-CT concepts within the C-CDA IG.

Please let us know if there are any further questions.

Thanks

Dragon

Hey Folks,

Based on the samples provided in the HL7 Examples Task Force page, it suggests we would not include the codes from the test data. Instead of including:
  1. No known Food Allergy (SNOMED-CT code =429625007)

  2. No known environmental allergy (SNOMED-CT code = 428607008)

  3. No known history of drug allergy (SNOMED-CT code = 409137002)

The samples indicate to include “Drug allergy (disorder) 416098002” from the Allergy and Intolerance Type value set with a negationInd=”true” to indicate no known drug allergies rather than using “No known history of drug allergy (SNOMED-CT code = 409137002)” by itself. When we use the three codes provided in the test data, the C-CDA R2.1 validator throws errors which I have attached. In which part of the final rule or the Common Clinical Dataset does it require including food or environmental allergies?

I will take this back to ONC and discuss it, certainly the environmental and food allergies could be removed as you suggest, since they are not specifically required by the rule.

Will look at the example task force recommendations before changing the other one.

Thanks

Thanks. Do you have a timeline for when you might have that conversation?

Already did yesterday and we are modifying the test instructions and the XML to only include the No Known Drug Allergies and follow the HL7 example.
