You should only submit the forms listed below for special situations. 1. Reporting Your Passport Lost or Stolen (DS-64)
- You should complete this form if you lost your passport or it has been stolen. To replace a lost or stolen passport, you must apply in person and submit Form DS-11 in addition to Form DS-64.
*Mailing times are not included in processing times. Processing times only include the time your application is at one of our passport agencies or centers. The total time to get your passport includes both processing and mailing times. It may take up to 2 weeks for applications to arrive by mail at a passport agency or center, and up to 2 weeks for you to receive a completed passport in the mail after we print it. Consider the total time it will take to receive your passport when you are booking travel.
Passports can only be renewed by mail. Online renewals are currently paused. Because it can take up to three months to process your application, it is important to renew your passport before it expires.
After you receive your renewed passport, if you change your name or find an error, you will need to get an updated one to reflect the change. The process, cost, and forms you need depend on how long you have had your current passport. Follow the steps from the State Department to change your name or correct your passport.
First-time passport applications have two types of basic fees, the application acceptance fee and the application processing fee. Passport photos and fees for faster processing are extra. Learn how to calculate fees and what you can use to pay for them.
Use the Online Passport Status System to check your application status if you applied by mail or in person. You may not be able to get a status update for 2 weeks after you apply or renew. During these weeks, your application and supporting documents are on their way to the Department of State. If you are eligible to renew your passport, please complete Passport Renewal Application Form DS-82 (see completion instructions on the form), and mail your completed renewal application directly to the State Department for processing. A minor's passport may not be renewed by mail; to renew a minor's passport, you need to submit a new, first-time passport application in person. An adult passport may be renewed by mail if it meets certain requirements.
Apply early for renewals as many countries require that passports be valid for at least 6 months after you enter their country. Learn how to properly prepare your application package and include all the necessary documents so your passport is processed on time.
If you need your passport faster for emergency travel or another special case, you can send your passport application via Priority Mail Express service and pay the State Department fee for faster delivery.
Passport applications for children under 16 have special requirements.
- Both parents/guardians must authorize the issuance of a child's passport. The best way to give consent is for both parents/guardians to go with the child when applying.
- Children need evidence of citizenship.
- Parents/guardians need proof of parental relationship.
- NOTE: Children's passports are only valid for 5 years. They are not "renewable": When a child's passport expires, you must apply for a new passport for them. When making a passport appointment for a child, please choose "New Passport Only" or "New Passport with Photo Services."
Applications to renew a damaged passport via mail may be accepted at our discretion if all the identifying information is still legible. If the photograph or identifying information is illegible, or the passport is significantly damaged, please refer to the information regarding Lost/Stolen/Damaged passports.
Most passport renewals for adults do not require an in-person appearance. Embassy Cairo now accepts routine adult passport renewal applications by mail for U.S. citizens in Egypt. Please see below for details of eligibility, application requirements, and where to send your application. In-person appointments for this service are not available. This mail in option is not available for children under the age of 16.
As a reminder, you must generate your application form online, print it, sign it, and mail it with the other required documents. We will no longer accept handwritten or typed passport application forms. The form must have a barcode printed in the top-left corner of the page.

The non-refundable application fee for an adult passport renewal using form DS-82 is USD $130.00. If you are applying for a passport and a passport card, total cost is $160. The fee for mail in applications can be paid through our online payment system.
You do not need to file Form I-912 for applications and petitions that do not require a filing fee. Other USCIS applications and petitions have fee exemption requirements for certain types of benefit requestors. In most cases, the USCIS form and instructions outline the fee exemption and submission requirements if a separate Form I-912 is not required. For a listing of alternate passport acceptance facilities, international travel guidance, and comprehensive passport information, please visit the Department of State. The Clerk of Court's Office provides passport application services by appointment only, from 8:00 a.m. to 4:00 p.m., Monday through Friday. Passport appointments are available on a limited basis at the following Maricopa County Clerk of Superior Court locations, and NOT available more than 60 days in advance:
Please bring the appropriate completed form(s) when you visit the Clerk's Office, or come prepared to complete the form(s), on your own, that are required for a passport book or card, depending on your needs. If you are uncertain as to which form you need due to your travel itinerary, please check with the Department of State; it is not the responsibility of the Clerk's Office to decide which document you will need for your specific travel arrangements. The Clerk's Office sole responsibility is to accept passport applications, documents and fees and forward these items to the Department of State so that they may process and issue you a passport from the information you provide.
The passport book fee for an applicant 16 years or older is $165 ($130 application fee payable to U.S. Department of State and $35 *Execution fee payable to the Clerk of Court). The U.S. Department of State and Clerk of Court fees are paid separately.
The passport card fee for an applicant 16 years or older is $65 ($30 application fee payable to U. S. Department of State and $35 *Execution fee payable to the Clerk of Court). The U.S. Department of State and Clerk of Court fees are paid separately.
When applying for the passport book and the passport card together, the fee for an applicant 16 years or older is $195 ($160 application fee payable to U.S. Department of State and $35 *Execution fee payable to the Clerk of Court). The U.S. Department of State and Clerk of Court fees are paid separately.
When applying for the passport book and the passport card together, the fee for a minor (under 16 years old) is $150 ($115 application fee payable to U.S. Department of State and $35 *Execution fee payable to the Clerk of Court). The U.S. Department of State and Clerk of Court fees are paid separately.
* The passport Execution fee is charged to passport applicants applying on Form DS-11 to recover the costs of executing the passport application, such as administering the oath, verifying the applicant's identity, and transmitting the applications. The execution fee must be paid at the time of application execution.
Two-way, overnight return delivery ($19.53) and expedited service ($60.00) is an additional total of $79.53 per application to the U.S. Department of State.. Plus, applicants will need to provide a United States Postal Service (USPS) Pre-Paid Express Flat-Rate mailing envelope or box large enough to contain your passport or multiple family passports. Please bring this item with you along with your passport application(s) IF you require two-way, overnight return delivery.
