This alert message occur in the second step:
"Please select an appointment hour before continuing!"
Occurs in firefox, chrome and IE. I think all is correct. I am so frustrated.
Some idea?

Testing...
I updated the php version ofthe script directory (with cpanel in php configuration) to 5.5 and works now.
