5. PROOF OF EXISTING BANK ACCOUNT. BANK STATEMENT OR TEAR SHEET SHOWING GOOD STANDING (CONFIRMATION OF THE ABOVE THROUGH A BANK) 

6. COMPLETED AUTHORIZATION TO VERIFY ON YOUR LETTERHEAD

7. CLIENT  INTAKE FORM COMPLETED
