Subject: Recruitment letter for cerebral palsy reseacher

This study is looking at how effective parents believe various treatments to be for children with cerebral palsy.  Participants must be parents/caregivers of children with cerebral palsy between the ages of birth and 5 years and 11 months.  Parents will be asked to fill out a survey of 24 questions and complete 3 subtest of the DAYC-2 assessment (physical, cognitive and communicative development).  The study can be done in person or over the phone, whichever is more convenient for participants.  Once parents have completed the study they will be give the opportunity to receive the results of the study and a $10 visa gift card.  Attached is a copy of the recruitment letter with all my information to contact me for anyone who is interesting in participating.
