Forms Integration

Are there any "Forms companies" like Jotform that integrates with DrChrono?

I want our patients to submit a form online and have it create a new patient in DrChrono.

Any help would be appreciated.  Thank you.

You can Post the request via jot forms using api and it will work. Or you can create a form on your website and with api you can achieve that.
