Dear All,

Please find the attached FP tool designed to capture the overall facility profile in context of FP service provisioning, Quality standards followed in Fixed/Camp and sample follow-up of FP beneficiaries to understand the Quality of FP Care and gaps. As we are practising the mechanism of addressal of gaps identified in facility tool, same shall be followed for FP related issues too.

Our team (BM/DQIS/RTS) are expected to initiate the process but the discussion shall be carried forward by the LR SN/ANM of the respective shift or Surgeon/MO who provided the FP services and MO I/C or MO to analyse underlying gap, corrective action. Compliance report of previous decisions to be discussed in weekly Clinical discussion meeting. All unresolved issues or non-compliance of decision taken in weekly clinical discussion must be addressed in monthly QI meeting of the facility, if required these may be addressed to higher level E DQAC ERQAC ESQAC to get support in resolving the same. The DQAC member (Govt) visit shall also be recorded in the tool.
