Dear All,

There was minor typo error in the earlier shared monitoring format, please find the corrected one, to be used to monitor FP camps during FP special drive from 15-29 Feb 2016.

List of 13 districts allocated to CARE by GoB and Informed Consent form to be used during Camp (later during fixed day as well) are also enclosed herewith for your reference.
