Internal Assessment of CHCs, Block PHCs and APHCs of Kayakalp Program by 15 August, 2016.

Dear Team,

As You know Kayakalp Award Program has been rolled out in FY 2015-16 in all district Hospitals of Bihar.

This is a government of India Initiative under Swachh Bharat Abhiyaan (SBA) for  Cleanliness, Hygiene and Sanitation in public places and hospitals.  

District Hospitals of East Champaran and Khagaria had won the 1^st and 2^nd prize respectively  in 2015.

For current FY:2016-17, Along with District Hospitals other hospital below DH have been included for this award program.

The District Hospitals, RHs, SDHs and CHCs who have completed their Internal Assessment will undergo Peer Assessment (List Is Attached Herewith) followed by State Assessment. For peer assessment the assessors are identified and they are completing the task by 2^nd August. The State Assessment is scheduled to complete by 25^th September so that the name of awardee can be declared on 2^nd October, 2016 (Gandhi Jayanti). For Peer Assessment your support is not required now.

 

But, we need to support at block level for internal assessment of CHCs, Block PHCs and APHCs. And this activity should be completed by 15^th August after which the Peer Assessment can be planned for those facilities who have scored more than 70% in internal assessment. Your role along with Block Managers is expected as Internal Assessors. RTS are required  to orient Block Managers and DQIS as internal assessors on attached checklist. The checklists are simple and easy to understand. Please call me if confusion arises. The checklists for CHCs, Block PHCs and APHCs are here attached for use. Peer assessment can be planned for these facilities after you submit the internal assessment. You can send a copy to me of internal assessment.

 

The total score for CHC:  500 marks (The block PHC who is recently upgraded to CHC should use CHC Checklist attached here)

The total score for Block PHC: 300 marks.

The total score for APHC: 200 marks.

 

Kindly go through the attached letter (Letter No. 2980, Dated: 28/07/2015 for understanding, facilitation and support to the respective blocks.
