Dear All,

In the Rhythm Workshop, Dr. Hemant Shah shared a template verbally with all DM to collect information of DH. I am sharing my class notes. Please incorporate, if I have missed anything:

A. Infrastructure.

B. HR

1. Specialist ( Sanctioned Vs Available)- Obs & Gyne, Paeditrician, Surgeon and Anaesthesist.

D. Is the paediatric ward functional? If yes, please highlight the dedicated ANM/Grade A/ Doctor in the paediatric ward including nos. of beds. Also the IPD load in paediatric ward....

E. Is lab services functional for 24x7? It includes all Lab Technicians wherever they are posted......

F. Is Duty roaster is in place? If Yes, is it followed?

G. Equipment & Consummable

1. LR- List of items which are not available or inadequately available or non functional or out of order?

2. NBCC- List of items which are not available or inadequately available or non functional or out of order?

3. SNCU- List of items which are not available or inadequately available or non functional or out of order?

4. OT-List of items which are not available or inadequately available or non functional or out of order?

5. Requirement Vs Availability

if I have missed anything, please incorporate it and collect this information from DH through DQIS or your self and send it to Dr. Prabir with a copy to me on or before 3 PM,16th September'2016 (Friday).
