Dear Team,

Following districts have not submitted their NQAS score card till date(Kindly check in the excel sheet attached herewith).

The districts are:

1. Banka

2. Bhojapur

3. Buxar

4. Madhepura

5. Madhubani

6. Nalanda

7. Patna

8. Saharsa

Kindly send them as soon as possible.

The other districts may validate their scores here.

In my previous mail (Sent from my Windows Phone Tue 6/9/2015 3:08 PM), I have clearly written you that:

2.Kindly use CHC checklist for FRUs below district level.

i.e. You should use CHC Checklist for SDH/RH/FRU-CHC which contain 12 departments.

But, some districts are using District Hospital Checklist for assessment of SDHs which contain 18 departments.

We are simply wasting our energy and time. I am sending you CHC checklist for FRUs other than District Hospital via www.wetransfer.com.

Please download it and use it.

3.Kindly use PHC checklists for non-FRU facilities.

PHC Checklist contains 6 departments which is attached here.

You should use it for assessment of Non-FRU CHC/PHC/APHC etc.

The DHS of other districts might have sent their scorecards to State Health Society.

But, I have not heard from our team members of other districts. Kindly ensure with DHS about accomplishment of the task and report the status here.

Dear District Managers,

Please refer the trail mail sent from Dr Prabir , SRU regarding NQAS certification. In addition to KPI of last 3 months (March April May), completed NQAS Score card and descriptive report (please refer khagaria report attached herewith) of each facility is required to be submitted to SHS latest by 12^th June 2015 for this purpose.

Most of the DQIS and RTS are well versed with these indicators and scoring process, I hope there may not be problem at DH. In most of the districts only one facility has been selected for the first phase, BM and DQIS may plan a joint visit plan to complete this. In few districts like Banka, Khagaria, Nalanda, Madhubani more than 1 facility have been identified, support of RTS to respective BM may be required to complete it on time.

I know you may take better decision by balancing the pre-planned activities & availability of skilled person currently in the district and so requested to communicate your team earliest to send all three docs by 10^th June to Dr Prabir with cc to me and Kaushik.

Dear Madam,

As you know, 48 facilities have been identified by the state in Phase-I for NQAS certification.

Care, BTAST and Unicef have been assigned to generate current score card for these facilities.

(i) Care has got responsibility of 31 facilities to generate score card(kindly find in excel sheet).

(ii) After assessment and scoring we need to prepare a document of individual facility.

A sample for reference is attached here(kindly find in pdf).

(iii) Then we need to prepare a folder for each facility which will contain (a) Score Card, (ii)Document (iii) KPI of last 3 months(March, April, May).

(iv) We shall share the folders with SHS by 12^th June, 2015.

For national certification , facilities need to score minimum 70%.

Hence , to improve the scores we need to focus on areas like LR, NBCC/SNCU, Maternity ward, Emergency, OT, OPD .

Dear All,

With reference to the commitment made by state during NPCC meeting held on 27.05.15 and also during review meeting convened by AS&MD ,NHM, GoI, this is to inform you all that for FY 2015-16 (1st phase) state has submitted request to GoI for conducting National Certification Audit of 48 health facilities from the month of July 2015 onwards.

In this regard we need to check their readiness and take immediate follow-up action for which your support is solicited. As discussed with you, the following activities needs to be conducted:-

1) Current scoring of 48 health facilities (List attached -annexure 1).

2) Report/document for each facilities . A sample document of DH khagaria is attached for reference .

3) KPI of last 03 months (March-may 2015).

For easy completion of the activities the facilities have been allocated to SRU/Care, BTAST & UNICEF (mentioned in list attached as annexure 1).

You are therefore requested to instruct you consultants at districts to complete the activities and ensure submission by 12.06.2015.

Note:- For national certification , facilities need to score minimum 70%. Hence , to improve the scores we need to focus on areas like LR, NBCC/SNCU, maternity ward, emergency, OT, OPD .
