District Compile sheet_ Facility monthly reporting with standard Facility name

Dear All,

Once again I am sharing the district compile sheet of Facility monthly reporting with revised drop down list of facility name. Rest of the format is same , no change at-all. As you aware we standardise the facility name to maintain uniformity in different reporting (MIS, FIS etc), accordingly the same has been incorporated in the compile sheet. Similarly we will use the same name in our communication (internal) or referring the facility under different groups like FP Comprehensive sites, PPIUCD sites, AMANAT facilities etc.

I received feedback from different districts on non-availability of guidelines of clinical discussion at BM & DQIS level and they are facing difficulties in conducting it. The Guidelines along with checklist (Hindi version) have been attached with request to share these with BMs and DQIS ONLY AFTER PROPER ORIENTATION of their use.
