The income disparity indicators are different than those used in previous scorecards; hence, these disparity rankings are not strictly comparable to those published previously. For some indicators, we combined multiple years of data to ensure adequate sample sizes for stratified analysis.
Performance Metrics. The 58 metrics selected for this report span health care system performance, representing important dimensions and measurable aspects of care delivery and population health. Where possible, indicators align with those used in previous scorecards. Several indicators used in previous versions of the State Scorecard have been dropped either because all states improved to the point where no meaningful variations existed (for example, measures that assessed hospitals on processes of care) or the data to construct the measures were no longer available (for example, hospitalizations for children with asthma). New indicators have been added to the State Scorecard series over time in response to evolving priorities or data availability (e.g., measures of COVID-19 vaccination status and medical debt).
As in previous scorecards, if historical data were not available for a particular indicator in the baseline period, the current-year data point was used as a substitute, thus ensuring that ranks in each time period were based on the same number of indicators.

Scorecards display a summary of a single metric. Scorecards are commonly used to visualize key performance indicators or some other variable that measures the relative health or performance of your business or area of activity. For example, a scorecard can summarize total sales, average bounce rate, count of ad impressions, maximum hold time, minimum failure rate, etc.
You can add a sparkline to a scorecard chart to visualize your primary metric's trends over time. This project was initially called "Security Scorecards" but that form wasn't used consistently. In particular, the repo was named "scorecard" and so was the program. Over time people started referring to either form (singular and plural), with or without "Security", and the inconsitency became prevalent. To end this situation the decision was made to consolidate over the use of the singular form in keeping with the repo and program name, drop the "Security" part and use "OpenSSF" instead to ensure uniqueness. One should therefore refer to this project as "OpenSSF Scorecard" or "Scorecard" for short.

The 2021 National Academies of Sciences, Engineering, and Medicine (NASEM) report Implementing High-Quality Primary Care: Rebuilding the Foundation of Health Care proposed the development of a scorecard to better monitor and ensure accountability for progress toward high-quality primary care in the United States. This first national primary care scorecard finds a chronic lack of adequate support for the implementation of high-quality primary care in the United States across all measures, although performance varies across states. To identify the metrics ultimately constructed for this scorecard, the authors began with an environmental scan and meetings with key stakeholders. (See Appendix A for a summary of the meetings.) To validate findings and measurement strategies, scorecard authors engaged with members of the scorecard advisory committee, NASEM committee members, and experts in the data sets being used. Where appropriate, measures were calculated using publicly available data to allow for easier reproducibility by stakeholders in the future. In some instances, particularly when workforce data were needed, the publicly available data did not produce an accurate measure and proprietary data sets were used.
Despite the robust set of measures presented in this scorecard and the accompanying dashboard, there are many gaps in existing data sets that limit the ability to provide an exact evaluation of the health of US primary care. Data limitations notwithstanding, this report can serve as a guide for state and federal agencies, private payers, and other stakeholders invested in strengthening primary care and measuring progress. The detailed methodology section in Appendix B explains the measures, data sources, and limitations, to facilitate replication of these metrics year after year.
