How To Add Custom Header in Pdf

hello guys,

i am newer in dompdf my site is like this http://joomla5.guru99.com/what-is-accounting.html at the footer there is create pdf button which create my pdf but

 i need some manual changes in my PDFs that's like if i download PDF it should be a logo or header of my website in to it like this http://screencast.com/t/jUOzqganC7

plese help mr to do this changes into it
