dochawk's bk now builds the new Statement of Intentions

I managed to overlook this one until I filed a couple of cases . . .

It now builds the new statement of intentions.

And it's a particularly stupid, wasteful form . . . rather than the
idiotic 1 & 1 continuation pages, it puts as many pages as it needs to
continue Part A, then does part B, with as many pages as needed.

Also, for part A (secured debts), it can take arbitrary intentions
that you type in, and has hotkeys for surrender, reaffirm, redeem, and
retain & pay.
