Upload "Failed" message and Error 400

I'm getting the message "Failed" when I try to upload images. And when I look in the console, the error I'm seeing is:

Failed to load resource: the server responded with a status of 400 (BAD REQUEST) http://mydomain.org/admin/filebrowser/upload_file/?csrf_token=5zSUnPV6L8vr0v4D…151-senior-man-considers-his-choice-on-the-supermarket-shelf-seriously.jpg

This is a new issue, I'm not really sure what would be triggering it. In looking at 400 errors, the error means that the request is violating a server rule. Is this something that is being triggered by my setup? Has anyone else come across this?

No traceback. That's the weird thing. Just the 400 error.

Did you happen to find a solution for this problem? I'm getting this error on my production server, but locally the same code works fine.
