Cross-references are picking up the wrong table and figure numbers

In a PDF transform, how do cross-references to tables and figures pick up table and figure numbers?

I've updated my transform plug-in (using tables.xsl and using code copied from commons.xsl) so that table and figure titles are displayed using folio-by-chapter format -- for example figure 1-1, 1-2, 2-1, etc. But the cross-reference text still shows the figure number as if I were using the defaults -- for example, figure 15 instead of figure 2-1.

I need to know where to fix the numbers so that they're picked up correctly for the cross-references, not just for the table and figure titles.

I'm using DITA OT 1.8.
