Reusing a topic multiple times in a bookmap breaks html/webhelp navigation/breadcrumbs - fix?

Hi there,
We have several different use cases in which we want to be able to repeat a topic in the same bookmap.  However, doing so does not work for HTML multiple file output.  The navigation doesn't work.  This issue is very similar to the situation discussed in these issues from a couple of years back:
When I've asked about things in DITA authoring user groups, there isn't a satisfactory response.  Basically all were variations on "create additional topics and conref the guts of the topic you want to repeat into them."  PDF seems to result in usable output, but not HTML/webhelp..
It seems to me like this is really an output issue and should be solved there.
So is there already a solution to this that works?  We're running OTK 2.2.

An update:
I've since found the OTK parameter "force-unique" and hope to test with it set to true, but wonder if it won't work if we have constrained the @copy-to attribute from the topicref attribute in our DTD.  Is this done automagically during the transform, or does it rely on @copy-to being specified in the XML source?

IIRC force-unique will take @copy-to into consideration when determining which topic references need to be rewritten to make each unique.
