Modem crash - (Hamlib error)

Long story short - was helping a fellow ham install LOG4OM and thought we had things working but now when he starts the modem on FreeDV he gets a "Couldn't connect to Radio with hamlib" error. (Full picture of error message attached).

He is running Windows 11 with a Elecraft K3. Tried a complete uninstall and reinstall of FreeDV and received the same error.

Any thoughts on what to do to fix this would be appreciated.
