PMJAY (Ayushman Bharat) - Activation of deactivated ID / exam completion deadline for active PMJAY

Dear Sir,
For PMJAY (Ayushman Bharat) deactivated Operator / VLE IDs, kindly fill out the below-mentioned Google Form for re-activation:

Once your ID is re-activated, you are required to complete the PMJAY exam on or before 15-01-2026.
Please note that the exam completion deadline for active PMJAY Operator IDs has been extended till 15-01-2026. All active operators must complete the exam and obtain the completion certificate. Only after successful exam completion will you be permitted to carry out e-KYC and Ayushman card delivery for beneficiaries.
