We recently got a chronic pain patient that has been through three surgeries for the upper cervical spine, first for Arnold-Chiari then two additional for stabilization and pain.

Through examination we’ve come to hypothesize that a portion of her pain (left sided severe headaches) is from dural/nerve tension resultant from adhesion created by the surgical procedures.  Movement of the limbs can lead to changes in her neck and headache symptoms from tensioning of the peripheral nerves and cord.

We frequently treat peripheral nerve entrapments and adherent nerve roots but this one is beyond my experience and expertise.  Is anyone familiar with any specialists regarding therapy for possible remodeling/flossing of dural adhesions?  Not sure if it even exists, but just want to seek out every chance possible for this patient to get the care she needs.

Thanks so much.

Hey Brian,
I am finishing up my diploma in Mckenzie (I just have to take the exam Oct 2012) and see patients like this. I would love to take a look at this patient for you. I am in the WhiteRock Lake area. I am out of town next week, but will be back in the office on monday March 12. Just let me know.

Brian,

There are some dural glide techniques that can be useful for that area. You can give me a call and I'd be happy to discuss those techniques and see if they may be helpful for your patient. Some of them are probably just a different approach of what you are already doing.

Sounds like maybe a task that needs to include a neuro-opthalmologist. I believe there is one located near Mockingbird & 75 in Dallas.
