Group,
I have a converted runner that is averaging 7-10 miles a week running
short distance events at a 7:45 pace complaining of continued Left
posterior tibial strain at the mid-tibia. On exam the maximum point of
tenderness is on the tibia itself. He has tried several running shoes,
is a neutral runner. No imaging studies have been done yet because
he's "scared" of the results (typical runner). I have tried him tight
and medium compression sleeves. Any further suggestions, aside from
imaging?

Need to look if he has some compensation patterns. I have had some patients weight bearing more medically or laterally that causes problems, weakness in the hips as well. My guess is that he probably may have a lot if soft tissue issues all long the that area as well that might need to be worked out. I hope that helps.

I concur with Stefani and would like to add looking at the specific mobility of the hip, crural, and subtalar joints.
