Request for Proposals

Human Centered Design and Strategic Outreach Materials

The Primary Care Coalition (PCC) is contracted with the Maryland Department of Health (MDH)

to lead the quality improvement component of the Integrated Maternal Health Services (IMHS)

Project. This project is funded by the federal Health Resources and Services Administration

(HRSA), and PCC was awarded a four-year grant from MDH to coordinate the integration of new

digital versions of the Maryland Prenatal Risk Assessment (MPRA) and Postpartum Infant and

Maternal Referral (PIMR) forms into workflows of inpatient and outpatient providers, local

health departments (LHDs), Administrative Care Coordination Units (ACCUs) and Managed Care

Organizations (MCOs) in selected service areas across Maryland.

The service areas for this project include the D.C. Metro region, eight Eastern Shore counties

(Cecil, Kent, Talbot, Caroline, Dorchester, Wicomico, Somerset, and Worcester), and the

Baltimore Metro region.

During Year one of the project (July 1, 2024-June 30, 2025), PCC mapped the current perinatal

risk assessment environment via a Knowledge, Attitudes, and Practices (KAP) Assessment, using

data from key informant interviews. PCC also created two oversight groups:

• The Perinatal Risk Assessment Working Group (PRAWG), comprised of all IMHS partners

that guides the project while also exploring opportunities to incentivize or remove

barriers to the use of this care coordination system, with a goal of increasing its use

across the state

• The Patient, Family, and Caregiver Advisory Group (PFCAG), which ensures that those

who have used perinatal care services are active participants in IMHS initiatives and that

their voices are heard.

PCC also established pilot projects at several clinical sites to implement the digital MPRA and

PIMR forms.

In Year two (July 1, 2025-June 30, 2026), PCC’s work is focused on continued technical

assistance for implementation of the digital forms at participating clinical practices, improving

clinical workflows using the digital forms, engaging the PRAWG and the PFCAG to enhance

perinatal care coordination in Maryland more broadly, recruiting individuals for the PFCAG,

and updating the KAP assessment to include any progress or changes from the beginning of the

project. PCC is also recruiting individuals to serve as regional clinical dyads and is onboarding

Baltimore City clinical practices.

Scope of Work

PCC’s proposal for this project includes contracting with a Human Centered Design Consultant

in Year Two of the project. PCC is searching for a firm that will:

• Update the KAP assessment by:

o Performing new interviews with key informants to collect information on what

has changed from the baseline assessment completed in January 2025

o Conducting initial interviews with Baltimore City partners and analyzing the

results

o Creating an updated report for PCC and MDH, and a presentation on the findings

to be shared with the PRAWG and other IMHS stakeholders

• Identify and interview high-volume sites that are not yet participating in the IMHS QI

project to understand their submission and referral capacity. Prepare a report for PCC

that includes the data collected and an analysis of the findings.

• Identify, survey, and/or conduct interviews with clinical practices within the IMHS

regions that are not yet participating in the IMHS QI project to quantify referral

challenges and gaps in community-based services. Prepare a report that includes the

data collected and an analysis of the findings

• Provide support to PCC staff in recruiting members for the PFCAG, including creating

recruitment materials and brainstorming who might be interested in participating.

Create opportunities for PFCAG members to participate in the IMHS QI project,

including facilitating PFCAG meetings, developing discussion topics for PFCAG members,

and coordinating Spanish-speaking facilitation, note-taking, and translation for

individuals who would benefit from a Spanish-only discussion.

• Design strategic outreach materials regarding the IMHS QI project, including:

o Recruitment materials for regional clinical dyad members

o Recruitment materials for PFCAG members

o Informational materials for perinatal care providers on the IMHS project,

including the goals of the project, the processes and procedures that are being

promoted, and the importance of participation in Maryland's perinatal care

coordination system.

o Informational materials on the IMHS project for patients who qualify for the

MPRA and PIMR about the digital referral processes and the services that are

available to patients through Maryland's perinatal care coordination system.

o Initial concepts for the design of digital toolkits for implementation of the digital

MPRA and PIMR forms into clinical practice workflows as the programs scales up

in subsequent project years

Proposal Submission

December 5, 2026. Proposals must include:

• Description of the firm’s capabilities and past performance completing this kind

of work;

• Description of firm’s approach to human centered design in a public health risk

assessment environment, KAP assessments, design of recruitment materials for

partners and participants for similar projects, creation of reports and

presentations for presentation of data analyses and findings, meeting

facilitation generally and for individuals who are not comfortable

communicating in English, and design and of various strategic outreach

materials and digital toolkits.

• High-level work plan including recommended measures of success and timeline

of submission of deliverables; and

• A budget proposal not to exceed $100,000

Evaluation of Proposals

Proposals will be evaluated by PCC staff and will be scored out of 100 points using

evaluation of the criteria as described in your proposal below:
