I am using the Premier Connect service and dial 0844 0416662.
It appears to want to negotiate a higher speed but after a number of
changes in negotiation tones it always connects at 33.6K.
Any ideas?

Firstly, demon.tech.modems is probably a better place to ask, I have
cross-posted and set a follow up.
OK, now for the questions:
Have you downloaded and installed all relevant patches, driver
updates, firmware updates etc for the modem?
Has the modem ever connected to demon at > 33k6 in the past?
... On the same phone line?
Has the modem ever connected to another ISP at > 33k6 in the past?
... On the same phone line?
Call BT Faults and ask if the line is connected through a DACS unit?
