Harris Health's pharmacy staff can sign you up for patient assistance programs to get free medicines from drug companies. You will be asked to sign the Medication Assistance Program (MAP) Consent and Authorization Form (Form #283233) that tells Harris Health to share your personal health information and sign any forms that are needed for you to get free medicine.
And you need a copy of one proof with a picture on it:

3. Gross income for the past 30 days for you, your husband or wife and children over the age of 18 who are living with you. As a new requirement for completion of your Harris Health Eligibility, every household member over the age of 18 must sign and date on the application to allow Harris health to check TWC information.
Cash income, dividends and royalties, rental property, alimony, workmen's compensation, military pay and allowances, current check stubs, child support documents, current IRS 1040/1040A tax return (all pages) if self-employed, Harris Health System Statement of Self Employment Income Form (English, Spanish, Vietnamese) if no tax return is filed, Harris Health System Wage Verification Form (English, Spanish, Vietnamese) (for cash and personal check wages only), Social Security award letter, Retirement award letter, Veteran Affairs letter or check, Agency letter, Income on SNAP form TF0001, unemployment benefit records or Harris Health System Statement of Support Form (English, Spanish, Vietnamese) if no income.
7. If you have Medicare and are eligible for Harris health System Financial Assistance Program:
You must fill out a Medicare Asset Form (English, Spanish, Vietnamese) and show proof of your current resources and liabilities (all pages of bank statements, credit card bills, loans, etc.).

PMJAY or Pradhan Mantri Jan Arogya Yojana is a flagship health insurance scheme by the Government of India. Also known as Ayushman Bharat Yojana, it provides financial security to the beneficiaries against hospitalization expenses in case of a medical emergency. It has been designed to cover the poor, vulnerable and the economically weaker sections of the society.
The PMJAY scheme comes with a maximum sum insured of Rs 5 lakh per family. Besides the hospital treatment cost, the scheme also covers the cost of diagnostic tests, medicines, pre-hospitalization expenses, COVID-19 treatment and post-hospitalization cost for almost all secondary and tertiary healthcare procedures. It also includes nearly 1400 expensive treatments, such as knee replacements, skull surgery, etc. The follow-up treatment expenses will also be covered under this scheme.
Moreover, the PMJAY scheme also offers cashless hospitalization treatment at network hospitals across the country. To be eligible for cashless treatment, the beneficiaries will be issued an Ayushman Bharat Golden card. The PMJAY or Ayushman Golden card is an e-card that needs to be shown at the network hospital to avail cashless treatment.
Not everyone is eligible for the Ayushman Bharat Yojana. Only people whose names are listed in the data provided under the Socio-Economic Caste Census (SECC) of the year 2011 and have the Rashtriya Swasthya Bima Yojana card are eligible for the PMJAY scheme. Thus, before applying for the Ayushman Bharat Yojana, people should check their names in the SECC database first.
Ayushman Bharat Yojana or PMJAY is considered one of the biggest health insurance scheme across the world. It covers the hospitalization expenses incurred by people belonging to the poor, vulnerable and economically weaker sections. They can also avail cashless treatment facility under network hospitals, provided they have the Ayushman Bharat Golden card. Starting this summer, UnitedHealthcare is eliminating nearly 20% of current prior authorizations, as part of a comprehensive effort to simplify the health care experience for consumers and providers.
These code reductions will begin in the third quarter and continue through the rest of the year for most Commercial, Medicare Advantage and Medicaid businesses. The Golden Eagle One Card Office oversees the printing and copier services on campus and issues the Golden Eagle One Card, the ID card for students, faculty, and staff. Our Office manages the copier and printer fleet programs, averaging between 56,000 and 78,000 copies each semester. We print about 3,900 student ID cards each term. One Card incorporates the use of Eagle $, door access, student printing, and meal plan to enhance the student, faculty, and staff campus experience.
