Dear All,
This is with reference to the pre placement offer extended to you for the position of "Graduate Engineering Trainee" in our organization and your subsequent acceptance of the same.
Enclosed herewith is the Medical Form and Guidelines for the Medical Tests.
Please note that the medical tests are to be carried out between 15-Jan-2014 and 15-Feb-2014, as the reports will be valid for the next 6 months from the tests being done.
1. The necessary tests need to be undertaken through any reputed MBBS Doctor at your end.
2. On joining, reimbursement of Rs. 750 only will be provided towards the expenses incurred for these Medical Tests.
3. You are requested to preserve the Medical Bills and submit the same at the time of joining to your respective Site HR at your allotted Manufacturing Site. Please note that no reimbursement will be provided without valid proof/bill.
4. Please ensure that all the details are appropriately and completely filled in the form provided and ensure that no blanks are left unfilled.
5. Also be very particular about the vision tests including one on Color Blindness.
6. You will also have to undertake an ‘Audiometry Test’ (which is not mentioned in the form) and send us the reports.
7. It is in your interest that you complete these tests at the earliest and send us the reports, PME form, and the X-ray sheet through courier/post, for our Company Doctor's scrutiny and clearance.
8. Important: Please keep a photocopy / scanned copy of the Form and Reports with you as a Back Up copy.
You may choose to collate all the reports of the selected candidates from your institute, and send it across, as a set, to us, through your T&P Office.

Your appointment will be confirmed subject to your clearing the Medical Test. You will receive the communication once you clear the Medicals.
