DDP report submission procedure?

Does anyone have any idea about procedure for submitting DDP report?

• which form do i have to fill?
• where do i get the form?
• which type of binding for the report? (hard cover or like the stage 1)
• Any other formality for the presentation??

I have report submission on monday and presentation on thursday. Kisiko pata hai to bata do.
